Service Processes
- Physician referred home care for individuals requiring the specialized services of a clinical specialist in neurology
Prior to physical therapy services a written physician’s referral stating:- “Physical Therapy Evaluation & Treatment”
- Duration and Frequency
- Diagnosis
- Direct Access Without Physician Referral – home care intended for short term interventions less than 30 days
- South Carolina allows direct access to physical therapy services. In the absence of a physician referral, physical therapy services can be provided for 30 days after the initial physical therapy examination. Thereafter, the patient must be referred to the physician and a referral as detailed above is required for continued physical therapy services.
- Individuals should verify with the insurance carrier that physical therapy services are covered and be pre-approved without a physician’s referral.

- Fees based on current Medicare reimbursement rate; sliding scale will be used to accommodate all individuals based on income
- Itemized service bill will be provided monthly so that individuals can submit to insurer for reimbursement
