Service Processes

  1. Recommend physician clearance to engage in exercise and or physical activity
  2. Required completion of the PAR-Q+: Physical Activity Readiness Questionnaire
  3. Vital signs and or fitness measurements will be taken based on established fitness goals
    • Blood Pressure
    • Pulse
    • Oxygen Saturation
    • Total Body Fat Prediction via Skin Caliper
    • Height & Weight

Payments

Electronic: Cash App, Zelle or Venmo
Check: payable to jvb physical therapy services, llc.
Due: First of month for monthly sessions (discounted rate) or at the beginning of each session
Cost: Fees based on Current Procedural Terminology (CPT) codes at the Medicare reimbursement rate for physical therapy services provided; a list of CPT codes and pricing will be made available prior to the first session
Receipt: Itemized service bill will be provided monthly or per session

About 3 months ago, I started working out with Jennifaye Brown, NeuroPT twice a week for only 30 minutes at 6:30am. When I say this was a completely different workout, I truly mean it.  With limited equipment (only a few handweights, stretchbands, and a chair) Jennifaye gives a full body workout! In 30 minutes, I’m sweating! She’s consistent, patient, explains the muscles used and exercises clearly, and always has a great attitude in the early am.  I couldn’t ask for a better workout! I feel the burn with each exercise, but not to the point of total exhaustion.  The exercises seem simple, but the results are complex. I feel great, look much better, and anxiously await the next session. She has catered the sessions to accommodate my travel schedule, injuries, and other activities.
